Parent Note/Official Excuse form for Deerfield Elementary

Dear Parents,

Due to the increasing number of parent notes being submitted with missing essential information, we have created a form to make it much easier for you to submit a parent note. This is the form which we prefer you complete any time your child is absent and you do not have a doctor’s excuse. It contains all of the essential information needed in order to process the note correctly.

You can print out the excuse, complete it and turn it into your child’s teacher or the front office within three days of the child’s date of absence. We truly appreciate your help in completing this form fully so that we will not be delayed in updating your child’s attendance records throughout the school year.

Supply Lists

Dear Parents,

I hope that everyone is having a wonderful summer! It’s hard to believe that June will soon be coming to an end and it will be time to think about purchasing your child’s school supplies.

You will find a school supply list for each grade level posted here. I know school supplies can get expensive, so I personally made a trip to a local Walmart to price these items. I would encourage you to take this list with you each time you go shopping to search for the best prices. Some of these prices were “roll back” items last week. As you know, the prices fluctuate from week to week. Typically, prices will drop much lower than what I have listed beginning in August.

Enjoy the rest of your summer!

Jan Malone

We have added a supply list tab for your convenience. Please click on the supply list tab, and then click your child’s grade level to see the supplies that are needed for the 2018/2019 school year.
